WIG CUSTOMIZATION FORM 

To receive the wig of your dreams please be sure to fill out the entire form below. The goal of this form is to get a clear depiction of the dream wig you desire.

In order to complete this form, you will need to do the following:

• You are required to include your personal information.

• You will need to have a vision for your wig.

• You will also need measuring tape to measure the perimeter of your head. On average, every head is everywhere between 20-24 inches.
